improve lives—one visit at a time. What You’ll Do as a Home Health
Physical Therapy Assistant: - Provide rehabilitative physical
therapy under the supervision of a licensed Physical Therapist,
following each patient’s individualized Plan of Care. - Assist
patients with exercises and mobility techniques to improve
strength, balance, coordination, and endurance. - Document patient
status, observations, and care provided promptly and thoroughly. -
Monitor progress and collaborate with the Physical Therapist to
adjust the plan as needed. - Demonstrate exercises or therapy
techniques for caregivers, home health aides, or family members who
will assist the patient. - Encourage patients and promote
independence and safety in the home. - Deliver care in alignment
with federal and state guidelines and professional standards. -
Build trusting relationships with patients and their families to
support consistent, high-quality care. About You: Qualifications –
What You’ll Bring: - Graduate of an accredited two-year Physical
Therapy Assistant program. - Licensed, certified, or registered PTA
as required by applicable state law. - In states without specific
licensure requirements, must be a graduate of an accredited PTA
program. - Current CPR certification required. - Strong
interpersonal and documentation skills. - Valid driver’s license,
auto insurance, and reliable transportation. Preferred Experience
(Not Required): - 1 year of experience in home health, hospital, or
outpatient rehab setting - In-home rehab services - Geriatric or
orthopedic rehabilitation - Collaboration in interdisciplinary care
